Clarity of keeping soap clear after an embed

I have been trying to make soap - pouring a clear layer, adding a piece of paper with writing, i.e. date and names and then adding a color layer to finish it off.

It looks great to begin with, but soon after the clear layer starts to cloud up and you cannot even see the writing anymore.

How can I prevent this from happening?

I have also tried to add a business card that has been laminated in clear plastic and embedding that and the soap still gets into the card and again, the clarity of the clear layer only lasts for a short time.

I would appreciate any help you can give me in this matter. Thank you.
